For residents who don't need long-term care, Chestnut Hill Convalescent Center offers a sub-acute care program designed to help them reach achievable recovery goals. Our comprehensive program is tailored to meet the medical and rehabilitative needs of a variety of individuals with different abilities.

Specialized care is provided by an attending physician and experienced therapists. The treatment team assigned to each patient includes a physical therapist, occupational therapist, speech therapist and recreation therapist, who develop individualized treatment plans.

Treatment may include:

• Balance training

• Patient/family teaching

• Muscle strengthening

• Transfer training

• Pain management control

• Gait training

• Assessments for orthopedic and home equipment needs

• Treatment for swallowing difficulties
