Together
with the Social Service department, our nursing staff participates
in discharge planning by providing recommendations for home care and
resident/family teaching, training family members to take care of
the resident after discharge.
Respite Care
Our facility offers a flexible care program. Respite care allows
your elderly family member to spend time in a caring environment
staffed by health care professionals 24 hours a day. He or she will
receive assistance with daily living activities, nutritious meals,
and, if appropriate, opportunities for rehab and recreational
activities. All regular medication will be administered and
supervised by our skilled nursing staff.

Hospice Care
When someone we love is approaching the end of life, our most
fervent wish is that he or she be allowed to die with peace and
dignity. At Chestnut Hill, we do everything we can to provide end of
life care that is compassionate and sensitive. Our hospice program
includes palliative care for pain management as well as family
support services. Clergy persons and social workers are available to
help with spiritual and grief counseling throughout the hospice
period.
